Abstract
Achieving a complete capsulorhexis in a hypermature cataract can be a challenge because of the raised intralenticular pressure and hence the convexity of the anterior capsule. Due to sudden release of the anterior intralenticular pressure when the first nick is made, an Argentinian flag sign can occur. To avoid this, we propose a simple, cost effective technique to ensure a complete single stage capsulorhexis. Here, a single nick is made with the cystitome, attached to an irrigation line, which provides continuous irrigation, thus maintaining the anterior chamber depth, and avoiding sudden reduction in the anterior lenticular pressure. Tipping movements are then carried out on the nucleus to decompress the posterior intralenticular compartment. Once the intralenticular compartments have been decompressed, the capsulrohexis is done in a continuous curvilinear manner. Hence, with the use of routine materials, we are able to achieve a successful capsulorhexis in a hypermature cataract.
