Abstract
Scleral Buckling is preferred to vitrectomy in paediatric patients to treat rhegmatogenous retinal detachment. A 4 year old child who presented with a post-traumatic retinal detachment with large breaks and localized PVR changes, retinal stiffening and sub-retinal sheets. We planned the child for sutureless scleral buckling with drainage using #240 Silicone Band and #279 Buckle. Despite apposition to the RPE, the posterior edge of the break needed additional support, due to the presence of fixed fold posterior to the break. Only a temporary augmentation of buckle was needed until the retinopexy permanently sealed the break. The posterior edge of the break was augmented using suprachoroidal viscoelastic buckle. Sodium Hyaluronate 1.3% was injected via an anteriorly placed full thickness sclerotomy using a 24G cannula. The sclerotomy was sutured after injection. Three months post-surgery, the break was sealed, augmented buckle had resorbed and retina was attached with improved vision.
