Abstract

43-year-old male presented with metamorphopsia. His BCVA was 20/20. Right eye had a subfoveal PED (1160 microns). He received intravitreal aflibercept injection following which his symptoms & height of PED reduced. However, 2nd aflibercept injection led to grade 3 RPE tear & subtotal bullous RD. His BCVA reduced to 20/200. He underwent 23-guage PPV, scrolling of RPE to normal position with help of DDMS, ILM peeling, FAE & post-operative PFCL tamponade. 5 days later, he underwent PFCL removal & SO tamponade. 1 month later, his BCVA improved to 20/40. Examination showed an attached retinal with complete SRF resolution & repositioned RPE. There were no post-operative complications like IOP spike, exaggerated inflammation or cataract progression.

Short-term PFCL tamponade prevents RPE from slipping back & helps iron out SRF completely. It can safely be used as a short-term post-operative tamponade in complicated RD without increasing the risk of any complications
