Abstract
The presenting author is a prolific phaco-refractive surgeon with numerous successful outcomes in challenging situations for last 17 years .Videos of challenging cases in Phacoemulsification has been presented in national and international podium .A case of iris coloboma always merits extra attention. Intraoperative Argentinian flag appeared in an intumescent cataract with iris coloboma and extended to the area of Iris deficit. It was difficult to retrieve such a rhexis. But with measured steps of nuclear management, altering the phacoemulsification fluidics the white cataract was emulsified in the bag. And with slow and steady surgery the case was completed successfully without any complication and a posterior chamber intraocular lens inserted. The patient gave an unaided vision of 20/20. The lesson learnt from the surgery was that we need to be vigilant for such cases but paradise is not always lost if we can control our fluidics and be patient with ourselves.
