Abstract

We present a case of a 28 year male who presented with sudden diminution of vision right eye for 2 days.His best corrected visual acuity was hand movement in right eye and 20/20 in left eye. Fundoscopy revealed diffuse retinal opacification at the posterior pole , cherry red spot at the macula with sparing of the cilioretinal artery territory in right eye. OCT macula showed diffuse inner retinal thickening of the temporal macula in the right eye. He had clubbing and harsh systolic murmur on auscultation.Laboratory examination revealed hemoglobin-22 g/dl, haematocrit-60%, serum iron-20microgram/L. 2D echocardiogram showed large ventricular septal defect with shunt reversal which was suggestive of Eisenmenger syndrome.Patient was managed in consultation with the cardiologist.

This case emphasizes the role of thorough evaluation in cases of CRAO specially in young adults for timely etiological diagnosis and management to prevent fatal complications like stroke and Myocardial infarction .
