Abstract
21 year old female admitted with complaint of diminution of vision in both eyes since 15 days which was painless, sudden onset rapidly progressive in nature. Known case of DM1 since 12 years and on insulin. History of diabetic gastroparesis 1 years back and diabetic ketoacidosis 5 months back for which she was treated.
On examination UAVA BE PL + with PR inaccurate in two quadrants, pupil 3mm, sluggishly reacting to light, rest anterior segment within normal limits. On dilated fundus examination of BE has features of Severe NPDR. On FFA features of PDR seen. BE OCT showed centre involving DME. Intravitreal injection Ranibizumab was given in both eyes with 1 day gap in between.
Ideally screening for DR in DM1is done within 3-5 years of initial diagnosis but in this case in spite of systemic complications of diabetes she was never screened for diabetic retinopathy which lead to blindness in this young patient which was rather preventable.
