Abstract
A 37 year old male presented with h/o electrocution and sudden onset vision loss in LE for 1 day.BCVA was PL+ in LE and 6/6 in RE.Pupil RAPD grade 3 was present in LE.Fundus examination showed temporal pallor in BE.A provisional diagnosis of optic neuropathy electric shock induced was made and the patient was started on IV Methylprednisolone 1g/day.On discharge the patient had PL+ in LE.One week later the patient presented with sudden onset vision loss in RE.BCVA in RE was PL+ and 6/60 in LE.Fields showed a hemianopic defect in LE.CT brain revealed pituitary macroadenoma with right cavernous and suprasellar extension impinging on the optic chiasm.Trans-sphenoidal microscopic excision was performed.On discharge the BCVA was PL+ in RE and 6/60 in LE.
A very rare coincidence of optic neuropathy triggered by electrocution and a pituitary macroadenoma causing compressive optic neuropathy necessitates the importance of investigating temporal pallor in young individuals.
