Abstract
A 44 year old woman with no systemic history presented with acute painless blurring of vision in the right eye for 1 week .On examination visual acuity was 6/6 p. Ant segment examination revealed grade 1 RAPD. Color vision was defective. HFA showed an inferior altitudinal field defect. Fundus examination revealed hyperemic disc edema with sectoral pallor seen superiorly. Left eye was normal. VEP showed reduced amplitude and increased latency in the right eye. A diagnosis of NAAION in a young female was made and considering the atypical presentation, patient was started on a trial of oral prednisolone and multivitamin tablets and advised to review after 2 weeks. No significant improvement was noticed. Further blood investigations were ordered which revealed hyperhomocysteinemia 32umol/L and anemia. Patient was started on folic acid and vitamin B complex tablets and physician referral given for the same.
