Abstract
A 17year old boy presented with profuse complaints of itching and watering since few months. On ocular examination, he had a visual acuity of 6/9 in both eyes, peri-limbal & conjunctival papillae , limbal stem cell deficiency and pannus were also seen bilaterally. A giant papillae was also seen on eversion of the left upper lid over the palpebral conjunctiva ,near the lateral canthus measuring about 3-4mm.The patient was initially treated with topical anti-histamines and steroids for a month. Despite long term treatment, the signs of chronic inflammation did not subside and this necessitated the use of stronger immunosuppressants , intra-lesional and sub-tarsal triamcinolone injection was given as well. The patient was followed up , his visual acuity improved along with decrease in signs of inflammation and the giant papillae evidently reduced in size. Hence, chronic refractory vernal keratoconjunctivitis can be well treated with a comprehensive approach using immunosuppressants.
