Abstract
42yr male presented with 7day h/o sudden vision loss, FBsensation,photophobia in LE.H/O B/L 16 cut RK 31 years ago after which he underwent Lasik 10 years ago. Slit-lamp examination: Corneal edema and stromal clefts in paracentral cornea with RK scars along with lasik flap scar. RE showed RK scars with lasik flap scar.
Scheimpflug imaging:post-RK cornea with severe ectasia RE.
AS-OCT:Multiple fluid-filled intrastromal clefts together with epithelial edema with DMD,suggestive of acute hydrops.
Pt treated with hypertonic saline e/d, topical beta blockers and topical steroids after which the corneal edema, descement folds decreased in LE
AS-OCT showed Persistent DMD with DM Rip with fluid pockets for which Pneumatic Descementopexy in LE wasdone
DM was partially attached resulting in pan-stromal scarring.
Patient rehabilitated with scleral contact lens.
Acute hydrops was possibly caused by corneal ectasia or underlying keratoconus which was aggravated by Radial Keratotomy and LASIK
