Abstract
A 17 years old female presented with complain of blurring of images after an episode of headache & vomiting. She had history of misalignment of both eyes(BE) since birth with insignificant birth history. Ophthalmic examination revealed BCVA of 6/6 with -0.5DS @90 degree, slight head tilt towards right, BE were in esotropia in primary gaze, limitation of abduction right eye with induced ptosis in right eye in abduction with widening of palpebral fissure of left eye in adduction and limitation of abduction left eye with induced ptosis in left eye in abduction with widening of palpebral fissure of RE in adduction. Fundus was unremarkable in both eyes.MRI brain &orbit was normal (to differentiate it from 6th nerve palsy). Symptoms of patient i.e.blurring of images resolved after giving the subjective correction which implied that symptoms were due to refractive error & presentation with headache&vomiting was purely coincidental. Hence,DRS may present with many variabilites.
