Abstract
DRS, a congenital ocular motility disorder characterised by marked limitation of abduction, variable limitation of adduction, palpebral fissure narrowing & globe retraction with overshoot on attempted adduction.We report 2 interesting cases of DRS with severe globe retraction and upshoot. In one of the cases, there is misinnervation to LPS which causes drooping of upper lid on adduction and elevation in right eye. LR recession with Y split was done in both cases as it is an effective procedure to correct upshoots with severe globe retraction in exo-DRS. Postoperatively we noticed that ocular deviation in primary position, upshoots, globe retraction were successfully treated. We also noticed, Y split resolved the drooping of upper lid as well suggesting mechanical restriction to be cause of induced ptosis on adduction. An optimal correction in globe retraction was obtained in both cases. We emphasize on efficacy of Y split of LR in alleviation of globe retraction in patients with DRS.
