Abstract
Ocular nail-gun injuries reported so far in literature were caused by nails, but we report a case of ocular trauma caused by wire ejected while using nail gun. A 32year old young male presented with sudden onset of severe pain and DOV in Right eye for 1 hour. He was using a nail gun without protective eyewear, when a piece of wire collation penetrated lower lid. On examination, his BCVA was 5/60 in R/E and 6/6 in L/E. A metal wire was seen penetrating right lower lid and globe around 10 mm away from medial canthus and 4 mm away from limbus respectively. X-ray orbit showed 3mm of metallic piece inside the globe. The metal wire was extracted under GA following the same track and 1*1mm scleral tear was sutured with 6-0 Vicryl. On POD-1, traumatic lens opacification was seen. After 2 weeks, R/E phacoemulsification with IOL implantation in sulcus was done leading to vision restoration with BCVA 6/9. This case highlights the importance of protective eyewear while using nail guns.
