Abstract
A 49 years old male presented with defective vision in right eye for 10 days. BCVA was 20/80 in the right eye and 20/20 in the left eye. On examination he had two pre retinal worms, one inferior to fovea and another temporal to fovea. He also had vitreous cells, cuffing of vessels. Cystoid macular edema (CME) was noted on Optical Coherence Tomography in right eye. A diagnosis of Diffuse unilateral subacute neuroretinitis (DUSN) with pre retinal worms was made and immediate laser photocoagulation to the worms was done. A systemic treatment of albendazole 400 mg once a day for ten days and oral, topical steroids in tapering dose was also given. In one month follow up BCVA right eye improved to 20/40 and CME resolved. In most of the cases the causative parasite species is not identified. Early identification, focused treatment, meticulous follow up are the lynch pin of treatment success in DUSN.
