Abstract
Combined choroidal and retinal detachment is a rare and complicated diagnosis presenting with many pre-operative, intra-operative & post-operative challenges. Herein, we describe a case of elderly male who presented with severely deformed globe & painless loss of vision in left eye. Anterior segment showed flat anterior chamber with multiple Descemet’s folds in cornea, position of intraocular lens was not clear & there was poor visualization of fundus because of non-dilating pupil and vitreous hemorrhage. B-scan confirmed combined choroidal & retinal detachment along with vitreous hemorrhage. Right eye was normal pseudophakic. Inserting an infusion canula for pars plana vitrectomy, in such a deformed hypotonus eye was the main surgical challenge. Decision whether to pre-treat with steroids, which approach to follow intra-operatively and avoiding post-operative complications were our concerns. We aim to present the challenges faced, and our approach of management of such a case.
