Abstract
A 19 years lady presented with sudden loss of vision in left eye since one month. She had no systemic symptoms. The vision was 6/9 in RE and counting fingers ½ foot in LE. RE showed vitreous cells and several areas of periphlebitis with candle wax drippings in the midperiphery . LE showed anterior vitreous cells, a fibrovascular membrane covering disc and macula with subhyaloid hemorrhage obscuring most of retina. FFA RE showed hot disc, staining and leakage in areas of phlebitis and LE leakage from the fibrovascular membrane. Laboratory findings were ESR(64mm/1st hour) and angiotensin converting enzyme of 230.41( normal range 8-68). Montoux test, TPHA and anti-nuclear antibody were negavtive. X ray chest and serum calcium were normal. HRCT was not conclusive. Patient underwent LE pars plana vitrectomy during which several sclerosed vessels were observed. Patient was diagnosed as ocular sarcoidosis and was started on immunosuppressive therapy and is under follow up.
