Abstract
We report 3 cases where unilateral difference between retinoscopic and subjective refraction was noted during preop evaluation before refractive surgery.Rest of the examination was normal.On cycloplegic refraction,it remained the same which did not change even after 2 weeks of cycloplegic treatment.At this point,macular OCT was ordered which showed Altered Macular Contour (AMC).Retinoscopy was done by changing the angle according to altered contour and it corresponded with subjective refraction.All 3 patients underwent Lasik based on subjective acceptance without any refractive surprises.
Discussion:AMC is characterized by convex elevation of macula.This elevation is negligible on fundoscopy or USG,but can clearly be detected via OCT.Retinoscopy reflex falling on relatively anterior,adjacent area of macula compared to fovea lead to lesser retinoscopic values in our cases.
Conclusion:Unilateral retinoscopy value less than subjective acceptance,one can consider AMC.
