Abstract
Central serous chorioretinopathy (CSCR) is characterized by serous detachment of neurosensory retina and/or the retinal pigment epithelium. It is due to leakage of vascular fluid through a focal leak in the RPE into the sub retinal space due to choroidal vessel hyperpermeability. Risk factors include type-A personality, pregnancy, and oral corticosteroids. We present a case of resistant CSCR who was on systemic steroids for SLE from past 10 years who responded to spironolactone therapy (the 3‘S’).
A 42-year-old lady presented to our OPD with a complaint of diminution of vision in right eye since 45 days, Fundus examination and OCT revealed CSCR. The patient condition warranted continuation of steroids hence she was treated with Spironolactone 25mg once daily, after due investigations. Serial follow ups revealed resolution of CSCR and improvement of vision. Spironolactone therapy as a novel approach is beneficial in patients with resistant cases of CSCR associated with risk factors.
