Abstract
AIM: To document cases presenting with ocular manifestations as first features of leukaemia. CASE-1: A 4 year old child presents with no PL, axial proptosis, exposure keratitis of the right eye. CT Scan and MRI reveals retro orbital mass encasing right optic nerve and extending upto cavernous sinus and intracranially into anterior temporal region. PBS shows atypical blast cells suggestive of acute leukaemia. Bone marrow smear shows 90% myeloblasts. Flow-cytometry confirms AML-M2. Treated with BFM-87 chemotherapy protocol. CASE-2: A non-diabetic, non-hypertensive 58 year old male presents with dimness of vision of both eyes for 4 months. VA being 6/12,N10 (OD) and 6/60, N24 (OS). Examination reveals multiple dot blot haemorrhages, macular edema and NVE on right eye. Vitreous haemorrhage, TRD and NVE on left eye. Immediate PRP 3 settings and accessible PRP was advised for both the eyes. PBS shows CML like picture. Bone marrow smear shows myeloproliferative disorder favouring CML.
