Abstract
Peripheral ulcerative keratitis (PUK) is an inflammatory disease of the juxta limbal cornea associated with crescent-shaped corneal stromal thinning, an epithelial defect, with inflammatory cells in the corneal stroma. It has both autoimmune and infective etiology . Rheumatoid arthritis (RA), granulomatosis with polyangiitis (GPA), systemic lupus erythematosus (SLE) are some common autoimmune causes. We present two case reports of Peripheral ulcerative keratitis secondary to herpes simplex infection which mimicked as infective corneal ulcer. A diagnosis of PUK secondary to herpes simplex virus was suspected after their routine , autoimmune work up and corneal scraping culture came back negative. Oral acyclovir and topical steroid was started and patient’s clinical status improved with resolution of infiltration and keratic precipitates. In PUK primary HSV infection should be considered as potential cause even though it is an extremely rare manifestation.
