Abstract
Purpose: Majority of keratoconus patients with healed hydrops cases require penetrating keratoplasty for visual rehabilitation. However, penetrating keratoplasty is associated with a risk of graft rejection. Methods: Herein, we report a 25-year-old Keratoconus patient who had bilateral healed hydrops. On examination visual acuity was finger counting in both eyes. Trial of right eye manual DALK with optical grade tissue backup was planned. Manual DALK was the only option for healed hydrops patients. Results: Successful manual DALK could be done with good visual rehabilitation. Vision was 6/24 in RE. Post operative course was uneventful. Conclusion: Manual DALK is a viable option for healed hydrops patient though optical grade corneal tissue as a backup is essential as chances of perforation are very high.
