Abstract
An 8-year-old male presented with redness, blurring of vision & increased haziness in left eye (LE) for 1 week. The BCVA was 6/24 in LE. On examination, anterior cornea of LE was clear, deep stromal haze was present with vascularization extending from superior limbus upto 1mm of pupil. AC was quiet. A diagnosis of HSV deep keratitis was made & usual treatment started. Initially he improved but after 2 months, he stopped responding. He was investigated for TB, sarcoidosis, syphilis but reports did not point towards any specific etiology. He was then kept on CMC e/d only. ASOCT showed deep keratitis (191µm) with some endothelitis with total thickness of 571µm. After 6 months, he has now multiple NMCO, deep vascularization with regressed lipid keratopathy & is started on e/d lotepred-tobra * BD & Tab. Acyclovir 200mg * QID for 10 days and will be followed up to check response. This is an unusual case of non-responding deep keratitis in a child with no previous attacks or systemic cause.
