Abstract
PURPOSE: To report a unique case of keratitis
METHODS: A 52-year-old female presented to Eye OPD with pain, redness and diminution of vision in left eye for 5 days after trauma by a sugarcane leaf. Patient seeked immediate assistance at a local doctor where topical drops were given but her symptoms did not relieve, for which she wanted a second opinion.On slit-lamp examination, there was an ulcer 5mm x 5mm present at paracentral temporal quadrant, with dendritic pattern of infiltration in periphery with dry dirty white in centre with portion of thorn present at the centre of lesion. A dilemma was present about the viral or a fungal aetiology of the ulcer. Empirical treatment for both was started and was modified under close follow-up.
RESULT: Patient showed improvement.
CONCLUSIONS: Scraping should be sent for both KOH and Grams stain, no matter if the patient has taken treatment from outside. Negative fungal hyphae do not rule out fungal corneal ulcer.
