Abstract

Case Description

A 30-year-old male presented with multiple ulcerative erosions in upper and lower lids of both eyes. Tarsal conjunctiva showed diffuse follicular conjunctivitis along with mucoid discharge. Inferior aspect of bulbar conjunctiva was stain positive.

Anterior segment showed multiple corneal epithelial defects with adjacent stromal haze and dendritic ulcers in RE whereas LE showed superficial punctate keratitis along with dendritic ulcers. BCVA of the RE and LE was 4/60 and 6/6, respectively.

Conjunctival scrapings came positive for HCV-1 from both eyes for viral -PCR. Patient was treated with topical and systemic acyclovir along with cycloplegics and all the lesions healed completely within 2 weeks. BCVA improved to 6/12 in RE and 6/6 in LE.

This is a rarely documented presentation of primary ocular herpes and ophthalmologists need to be aware and vigilant of this, as one of the myriad manifestations of primary ocular herpes, thereby providing appropriate treatment.
