Abstract

46 years old diabetic and dyslipidaemic female with sudden, painless diminution of vision in the left eye of 1-hour duration. Her BP was 236/116mm Hg, left eye had grade 3 relative afferent pupillary defect with a BCVA of counting fingers at half a meter. The other eye had a normal anterior segment with a BCVA of 6/6p. upon pharmacological mydriasis, a dilated fundus examination revealed a normal fundus of the right eye and cattle tracking of retinal arteries of the left eye. a diagnosis of CRAO was made and a traditional emergency management o the condition was instituted- which included oral acetazolamide 250mg and aspirin 75mg, local instillation of eyedrops timolol 0.5%, ocular massage and carbogen inhalation. she was explained poor visual prognosis and admitted to ICU immediately for management of raised BP, blood sugar levels, and dyslipidemia. upon stabilizing and reviewing her the next day, she had a vision of 6/6p in both eyes with the restoration of normal retinal blood flow.
