Abstract
Physician referred a 26/F presenting with fever,malar rash and loss of appetite for a month complaining of blurred vision in LE for a week.She is K/C/O hypothyroidism and seizure disorder for 10 yrs.O/E BCVA OD-6/6,N6;LE-6/60,N24.Colour vision:RE-16/16,LE-0/16.LE-RAPD present with painful EOM motility.IOP was normal.Fundus showed RE-single CWS above disc(cotton wool spot) and LE-pale disc with ill defined margins,multiple CWS around optic disc ,venous dilation and tortuosity with severe arteriolar attenuation,serous exudative detachment involving macula suspicious of hypoperfusion retinopathy(LE>RE).Haematological,immunological investigations concluded to diagnosis of SLE(systemic lupus erythematosus)for which treatment started.She progressed to LE secondary optic atrophy(SLE retinal vasculitis sequelae)with BCVA-HMCF.This unforeseen SLE retinopathy and its sequelae of poor visual prognosis necessitates vigorous ocular examination and follow up for timely diagnosis and intervention.
