Abstract
Total Ophthalmoplegia after Covid-19 infection can pose a great diagnostic challenge A 38-year-old male patient presented with complete ptosis along with a headache. He was tested positive for SARS COV 2 a month before the symptoms. Visual acuity was 6/6 in both eyes. On examination, he had complete ptosis, total Ophthalmoplegia with fixed, dilated pupils. Corneal sensation was reduced. However, the fundus examination was normal. A provisional diagnosis of superior orbital fissure syndrome was made. Neuroimaging was inconclusive. Ethmoidal sinus biopsy was done to rule out invasive fungal sinusitis, which came negative for fungal hyphae. He was treated with oral steroids after which his symptoms improved significantly.
