Abstract
A 27yrs old male presented with pain & protrusion of right eye with progressive diminution of vision for 6months. On examination he denied PL in OD with BCVA 6/6,N6 in OS. There was significant fullness of Right Upper Lid on lateral aspect with “S-shaped” blepharoptosis. Limitation of movement was noted in OD lateral gaze. OS examination was insignificant. CT orbit revealed soft tissue density lesion 3.5×3.4cm OD with erosion of lateral roof of right orbit, frontal bone & greater wing of sphenoid and Intracranial extension. Bony secondaries were noted in right ramus & condyle of mandible. MRI Brain & Orbit showed hyperintense lesion involving right LR with adjacent frontal & temporal bone, s/o infiltration. Incisional biopsy over the swelling with HPE revealed Adenoid Cystic Carcinoma of Lacrimal Gland OD. He was managed with systemic antibiotic and analgesics. External Beam Radiotherapy was given resulting in remarkable decrease in swelling and symptoms. He is under regular follow up.
