Abstract
Introduction-Rhino orbito cerebral mucormycosis(ROCM) complicating COVID-19 cases has emerged as challenges recently. We report an atypical case of ROCM mimicking orbital abscess. Case- A 36-year-old male with history of COVID-19 presented with headache, decreased vision and swelling in right eye since 6 days. Examination showed restricted ocular movement in right eye with dystopia, mild conjunctival chemosis and ptosis. Diagnostic nasal endoscopy was insignificant. Contrast enhanced MRI showed a heterogeneously enhancing lobulated lesion in superior extraconal compartment in right orbit indenting upon extraocular muscles. The lesion was approached supraorbitally through a pinhole incision. Granulomatous tissue and pus were removed. Microbiological and histopathological workup confirmed mucormycosis. Early surgical removal and timely administration of amphotericin-B led to full recovery. Conclusion- Careful examination and early imaging are life saving for post covid mucormycosis.
