Abstract
Intracranial psammomatous meningiomas (ICPM) are rare subtypes of benign non-aggressive meningiomas. Radiologically they present as calcified lesions but not as multi-loculated ones.
We report a case of 16-year-old female who presented with proptosis in right eye for 2 years and diminution of vision for 2 months. Her BCVA was 6/6 in both eyes. On examination, RE non-axial proptosis was noted in downward direction with RAPD of grade 2. Fundoscopy revealed hyperemic disc with normal periphery. CT and T1W MRI revealed multiloculated hypointense lesion in right frontal region with multiple thin internal septations, suggestive of Aneurysmal Bone Cyst (ABC) of frontal bone. Excisional biopsy revealed whorls with plenty of psammoma bodies suggestive of psammomatous meningioma.
ICPM is not well reported due to its low incidence but those reported have shown high calcifications in radiological imaging in contrast to our case which showed no calcifications but multi loculations mimicking ABC.
