Abstract
Purpose: To present a case of B/L myopic strabismus fixus with B/L RD presented with Lens-induced glaucoma and to describe the pathophysiology, C/F and management for the same.
Case report: 57 year old female presented with the painless gradual diminution of vision in both eyes since childhood and inward deviation of both eyes since her pubertal age. But what brought her to the hospital was severe pain, redness, and watering in her left eye for the last 5 days.
O/E visual acuity in BE inaccurate PR, subluxated cataractous lens in RE, phacomorphic lens-induced glaucoma in LE. BE had >45 degrees esotropia with restricted ocular motility . An IOP was 42.2 mmHg in RE and 20.6 in LE. AL on A-scan was 32 mm and 32 mm in RE and LE resp. B-scan revealed B/L posterior staphyloma with RD.
Treatment: After reducing the IOP, we performed cataract extraction with PCIOL implantation in both eyes without any muscle/ extra-ocular surgery as there was no chance of gain of vision and binocularity.
