Abstract
Purpose: Iatrogenic intraocular foreign body detected on1st post-op day after vitrectomy. Methods: 58-year-old male with sudden diminution of vision OS. His BCVA was perception of light OD and 20/200 OS. Fundus exam showed a vitrectomized silicone oil filled eye with a severe macular pucker OD and rhegmatogenous retinal detachment with VH OS. Results: PPV OS was performed with silicone oil tamponade. BCVA on 1st post-op day was 20/40. Fundus examination revealed vascular narrowing and whitening with perivascular hemorrhages inferior to the macula, few pale centered hemorrhages and ‘a cotton wool spot’. On serial follow-up, the cotton wool gradually migrated inferiorly, the vessels re-perfused and the hemorrhages disappeared. Conclusion: The presence of cotton wool on the retinal surface post operatively was alarming.The sources of these ‘iatrogenic’ foreign bodies need to be identified and avoided to prevent catastrophic consequences.
