Abstract

Congenital X-linked retinoschisis involves splitting of the retinal layers and is usually a stable disease rarely presenting as retinal detachment (RD). A 1.5-year male with manifest exotropia was referred for right eye (RE) RD surgery. On examination under anaesthesia RE fundus revealed macula involving inferior RD with a thinned-out retina and multiple retinal holes. Diagnosis was made of congenital retinoschisis when the Left eye (LE) showed a spoke-wheel pattern at the fovea and inferotemporal schisis without any breaks. Repeated examination of the RE was done which showed combined rhegmatogenous schisis. RE was taken up for parsplana vitrectomy and LE was observed. Surgical challenges included posterior vitreous detachment in a child, identifying and treating all inner and outer retinal breaks and unfolding the outer retinal fold that was seen intraoperatively. Schisis RD can be easily misdiagnosed as simple RD unless both eyes are carefully examined.
