Abstract
90 years old known diabetic male presented with sudden increase in size of upper eyelid swelling associated with decrease in vision in his left eye since 3 months and right big toe ulcer since 1 month. Ocular examination showed left eye no light perception with upper eyelid ulcerated swelling covering the globe. General examination showed enlarged left sided preauricular and cervical lymph nodes with right big toe ulcer which was clinically diagnosed as diabetic foot ulcer. Blood sugar was within normal limit. Diagnosis of sebaceous cell carcinoma was confirmed on histopathology from lid swelling. Lymph node FNAC and right toe ulcer histopathology diagnosed metastasis of sebaceous cell carcinoma. CECT revealed systemic metastasis to lung, liver, bone and lymph node. Foot ulcer is rarely a presenting feature of distant metastasis of sebaceous cell carcinoma. This case is rare presentation of sebaceous cell carcinoma with metastasis to foot and internal organs like lung, liver, bone.
