Abstract

INTRODUCTION: To describe the clinical features, management and response of patients with scleritis associated with Granulomatosis with Polyangiitis(GPA).

METHODS: Demographic and clinical profile, and management of patients presenting to a tertiary eye care centre with scleritis secondary to GPA from 2003 to 2021 were analysed.

RESULTS: 19 eyes of 17 patients (15 c-ANCA positive) were included. 15 eyes had necrotizing scleritis, 2 diffuse anterior scleritis and 2 nodular scleritis. 14 patients were treated with oral steroids and cyclophosphamide of which 2 required additional methotrexate and 2 additional MMF. 13 patients received intravenous therapy – 4 methyl prednisolone, 6 cyclophosphamide and 3 rituximab. 3 eyes required scleral patch graft. 14 patients had good outcome anatomically and visually and 3 patients were lost to follow up.

CONCLUSION: GPA associated scleritis can have good prognosis when managed aggressively in the acute stage with adequate systemic immunosuppression.
