Abstract
Management of rock hard, brunescent cataracts in one of the most challenging case scenarios faced by a cataract surgeon. These cataracts are commonly associated with co morbidities like weak zonules, thin posterior capsule, poor endothelialcell count, poorly dilating pupil. Successful removal of hard nucleus is key to successful outcome in these cases. In this video we discuss strategy to effectively chop such nuclei using horizontal chop technique. We use Chop-Rotate-Chop strategy which involves chopping the nucleus through its entire diameter by rotation, before attempting lateral split of the lens fibers. We demonstrate that this technique is effective even in a patient with extremely loose zonules, which validates effectiveness of this technique.
