Abstract
Arteriovenous malformations (AVM) are congenital hamartomas which appear as a result of defect in vascular maturation. A 18-year-old male presented with painless, gradually progressive swelling over right upper eyelid since 7 years after incidental fall from a two-wheeler. Examination revealed a 2*2 cm soft, non-tender swelling in upper eyelid with pulsations and bruit and increased in size on Valsalva. There was no associated signs of inflammation. CT angiography showed hyperdense lesion in right frontal region with arterial feeders from facial artery. Both fundus showed peripheral bony spicules, mild arteriolar attenuation and normal optic disc. A working diagnosis of right upper eyelid AVM with bilateral retinitis pigmentosa was considered. He was managed with trans-arterial embolization in two sittings by interventional radiologist and counselled regarding fundus findings. This report implies pivotal role of multidisciplinary approach in management of such cases.
