Abstract

A 20 year old male with both eyes vernal keratoconjunctivitis on Olopatidine eye drops since 5 years presented with decreased vision of 2 months duration. BCVA was OD 1/60 and OS 6/18. Both eyes showed 360 conjunctivalisation of cornea with anterior stromal scarring in right eye more than left eye, suggestive of limbal stem cell deficiency. He underwent right eye allogenic simple limbal epithelial transplant with amniotic membrane graft. Topical Prednisolone, Ofloxacin, lubricant eyedrops and Tacrolimus 0.1% ointment were started post-operatively. One month later, remarkable regression of stromal scarring and vascularization was seen in right eye with BCVA 6/6. In this case, prolonged use of BAK containing drops could have added to the limbal stem cell insult. With minimal tissue, allogenic SLET can salvage vision in LSCD. Dramatic visual and ocular surface improvement can be achieved, provided inciting agents are identified along with timely intervention.
