Abstract
AIM:To highlight need for thorough systemic evaluation in a case of PUK. METHODS:Reporting a case of necrotizing sclerokeratitis at a tertiary eye hospital
RESULTS:A 62-year-old male,with no known ocular or systemic comorbidities, presented with severe pain and redness in his right eye since 1 month, triggered by vigorous rubbing of right eye following fall of dust particles into his eye.Patient was systemically investigated & found to be positive for proteinase 3 anti-neutrophil cytoplasmic antibody & raised erythrocyte sedimentation rate.A provisional diagnosis of granulomatosis with polyangiitis (GPA) was made.Patient was started on systemic & topical steroids along with cyclophosphamide after consulting the rheumatologist.The patient responded satisfactorily to treatment.
CONCLUSION:Ophthalmological manifestation may be the initial presentation in a case of GPA.This case highlights importance of extensive systemic workup to uncover potentially fatal systemic associations of PUK.
