Abstract
Case report: A 59 year old diabetic and hypertensive male presented with history of dimness of vision in Right Eye(RE) for 7 days. He had history of renal transplant and is on oral prednisolone (5 mg) daily. Patient had taken Covishield vaccine (1st dose) 8 days before onset of symptoms.
BCVA was 6/9 in RE and 6/6 in Left Eye(LE). IOP was 13 mmHg Both Eyes(BE). Fundus of RE showed disc edema and haemorrhage, dilated and tortuous retinal veins with dot and blot haemorrhage in all quadrants. Whitening of macula was seen superior to fovea (combined BRAO and CRVO). Fundus of LE was normal.
OCT RE – hyper-reflectivity of inner retinal layers superior to fovea, absence of intra or sub-retinal fluid. Angiography not done due to renal transplant.
Advice- To review with physician. Need for regular follow up explained.
Discussion: Combined BRAO and CRVO though rare, may be seen in patients with multiple co-morbidities. Covishield vaccine as an inciting event needs further evaluation.
