Abstract

Accidents are not rare during any anterior segment surgery. One of them is inadvertent release of OVD cannula during injections of solutions or OVDs resulting in intraocular injuries. We report a case of similar incident during injection of OVD in anterior chamber during a routine cataract surgery despite the use of Luer-lock syringe. This accident resulted in large posterior capsular rupture as the cannula hit the nucleus and ruptured the posterior capsule in jet within no time. The case was managed by converting into small incision cataract surgery and a PMMA IOL was placed in sulcus as the anterior capsular rim was unaffected. The post-operative refraction was -2DC at 180 degrees with BCVA of 6/12, N8. The accidental release of cannulas during any anterior segment surgery is very rare but may cause serious sight threatening intraocular injury. The surgeons should recognize this complication and know how to prevent the occurrence of this rare iatrogenic intraoperative complication.
