Abstract
Optic neuritis is an inflammatory, infective or demyelinating condition of the optic nerve. Its occurrence postpartum, during lactation is known as lactation optic neuritis.
A 21 year old female, 2 months postpartum came with complaints of sudden, painful diminution of vision in left eye, since 15 days. Visual Acuity (VA) OD – 6/9(p) OS- CF 1m. Anterior segment : Pupil OD – 4mm, reactive to light OS- 8mm, fixed, non-reactive to light. Extra Ocular Movements – painful in the downward gaze. Fundus showed temporal pallor in the left eye with normal disc. Immunological and blood workup was negative for autoimmune or infectious etiology. MRI Brain and Orbit showed hyperintense intra-orbital segment of left optic nerve with enhancement of left retrobulbar region suggestive of left optic neuritis. Treatment given- 1 gm IV Methyl Prednisolone for 3 days. Discharged after day 3 on oral dose 1mg/kg for 11 days. VA at discharge – 3/60 Snellens.
