Abstract
PURPOSE: Commotio retinae is present in about 30% of all eye traumas presenting for hospital treatment. Considering how the management of the case can have an impact on the recovery and visual acuity , all cases with a history of assault/blow to the eye should undergo a thorough anterior segment and dilated posterior segment examination.
PRESENTATION: A 25 year old male presented with a black eye following a blow to the right eye with a fist in a fight 2 days ago. On Examinaton he had 6/6 UCVA, pupillary reacton and anterior segment in both eyes. Dilated examination showed widespread Commotio retinae involving the posterior pole in the right eye and an unremarkable fundus in left eye. CT Scan showed periorbital soft tissue swelling and undisplaced fracture of the right lateral orbital wall as well as facial bones.
CONCLUSION: Urgent retina examination following a history of assault and regular follow up can help prevent significant and disabling vision loss in a few patients.
