Abstract
A 28 years old male came with a sudden onset blurring of vision in OD following a road traffic accident with head trauma. The BCVA at presentation was 20/125p OD and CFCF in OS. Patient gave history of poor vision in OS since birth. OD revealed presence of multiple cotton spots, flame shaped hemorrhages, cherry red spot with posterior pole whitening along with typical Purtscher’s flecken and perivenular clearing features suggestive of Purtscher’s retinopathy. OS showed presence of extensive myelinated nerve fibre layer involving the disc and the arcades at the posterior pole. He was administered 3 doses of pulse IVMP 500mg consecutively for 3 days and then on tapering doses of oral steroid for 1-month. The BCVA improved to 20/30 in OD after 3 doses of IVMP which subsequently improved to 20/20 at 2 weeks and remained stable at 6 months. Our case illustrates a dramatic response to IVMP in a case of Purtscher’s retinopathy with a drastic improvement in visual acuity as early as day three.
