Abstract

Introduction: Slit-lamp evaluation of post trabeculectomy bleb morphology can predict reasonably whether the bleb is going to be functional or failing, enabling early steps to maintain target IOP and prevent disease progression.

Material and Methods: 10 trabeculectomies done using standard techniques without anti-metabolites. Patients followed up on Day 1, Week 1, Week 4, Month 3 and Month 6

Key considerations:

1. Bleb Height

2. Bleb Extent

3. Vascularity

Discussion: Post operative trabeculectomy bleb failure is a commonly encountered situation for glaucoma surgeons. It may occur due to various reasons starting from preoperative, intraoperative and postoperative causes. Careful examination & proper documentation at each follow up visit by slit lamp can predict whether the bleb is going to fail or not so that necessary steps can be taken to maintain target IOP.

Conclusion: Signs of failing bleb

1. Excessive vascularization

2. Flat thick-walled blebs

3. Encysted with demarcation lines
