Abstract
Introduction
Diabetes mellitus is a rare but benign cause of Cranial neuropathy.Third cranial nerve is most commonly affected due to diabetes . Rarely, sixth cranial nerve palsy can occur.
Case report -We report a 48-year-old man, a known case of diabetes mellitus for 10 years, who presented with binocular diplopia, secondary to sixth nerve palsy with headache of one week duration. At presentation his blood sugar was uncontrolled. Neuro-ophthalmologic examination was normal except for unilateral impairment of abduction on the left eye rest ocular movements were painless and full . MRI imaging reveals orbital and seller pathology.
We had started oral NSAID with oral mecobalmin with gabapentin and regular weekly follow up.After 12 weeks diplopia was resolved and ocular movements were full and painless.
Conclusion
Six nerve palsy despite being a rare entity in diabetes mellitus, appears to be a serious problem from a diagnostic and therapeutic point of view.
