Abstract

A 17-year-old male presented with acute painful vision loss and pain in left eye (OS) following trauma with stone. Slit lamp examination revealed diffuse corneal edema, anterior chamber hyphema, anteriorly subluxated traumatic cataract with secondary angle closure with an IOP of 40 mmHg. The best corrected visual acuity (BCVA) in OS was perception of light. He underwent intracapsular cataract extraction with anterior vitrectomy along under mannitol cover. At 1st week follow up, an IOP of 18 mmHg could be achieved under topical AGM cover with an improvement of BCVA with +10DS to 20/80. Subsequently, a pars plana vitrectomy with secondary SFIOL was done for the dropped lens matter in the first surgery. An improvement of visual acuity to 20/60 with an IOP of 12mmHg was noted at 1 week, however fundus showing the presence of multiple sub-ILM hemorrhages features suggestive of ODR. This case illustrates the development of ODR following a substantial decrease in IOP post SFIOL.
