Abstract
A 71 year old female presented with periocular redness and watering in the left eye since 1 day. It was sudden in onset following a brisk episode of an excruciating unilateral headache. There was no history of fall or any sought of ocular trauma. On examination, there was periocular ecchymoses and total subconjunctival hemorrhage. The extraocular movements, intraocular pressure and the anterior segment were within normal limits. Orbital margins were intact on palpation. On fundus evaluation, vascular attenuation was noted with the rest of the fundus being normal. On systemic evaluation, she had a high blood pressure of 200/100mm Hg and was immediately started on antihypertensives Bleeding disorders, vascular or space occupying lesions of the brain have been ruled out. Patient resolved completely with the systemic control of hypertension. This case highlights the possibility of such an alarming presentation and also a simple methodical approach with systemic evaluation.
