Abstract
Sixth nerve palsy is rare in young adults and is not well studied. Recently Covid-19 has been implicated as a cause of Acute sixth nerve palsy.We had two young adults who presented with diplopia of 1 week duration.They had history of Covid infection 2 months back.On examination they had esotropia with limitation of abduction in one eye. Systemic examination did not reveal any abnormality.There was no evidence any acute infection.Visual acuity was 20/20 OU in both patients with unremarkable anterior and posterior segment examination.They were positive for SARS-2 antibody.However contrast imaging of head and orbit did not reveal any abnormality.All blood investigations were normal. They were treated with oral steroid and after 2 weeks there was complete recovery.The delayed damage appears to be mediated by the immune system as a post infective phenomenon. Our cases shows that delayed presentation can occur in Covid-19 and that it rapidly responds to steroids.
