Abstract

Chronic kidney disease (CKD) is a frequently encountered comorbidity in Ophthalmic surgeries. Since platelet dysfunction in CKD can cause unexpected bleeding episodes and thrombus formation, adequate preoperative precautions and frequent postoperative follow-up are needed to prevent unforeseen complications. We report a case of a 58-year-old female with chronic kidney disease on hemodialysis who presented with left eye panophthalmitis. Her bleeding time was normal, and she underwent an emergency evisceration. The intra-operative and immediate postoperative periods were uneventful. However, she presented on the second postoperative day with a huge clot filling the entire scleral cavity. We decided to treat the patient conservatively. The clot resolved spontaneously after two weeks, and the scleral cavity cleared after one month. Ophthalmic surgeons should work with nephrologists and anticipate delayed bleeding in uremic patients and monitor them closely in the postoperative period.
